This depends on your insurance plan. Most insurance plans require either a copay or coinsurance due by the patient for each visit. Other insurance plans also have an annual deductible which may be due. We can help supply you with this information when we verify your insurance on your 1st visit to our clinic. In cases where patients have no insurance or where their insurance has expired, the cost of each “self-pay” follow-up visit is $95. The charge for an Initial Evaluation is $135.
